Integrative Multicentric Deep Learning to Enhance Urine Cytology for Bladder Cancer Screening and Surveillance

Summary:

Bladder cancer is one of the most common and recurrent cancers worldwide, requiring frequent and lifelong monitoring after diagnosis. The current standard approach, cystoscopy, is invasive and can be uncomfortable, while urine cytology provides an important noninvasive complement. However, despite its widespread use, urine cytology remains inconsistent and inefficient. Variability in performance can lead to missed disease, unnecessary follow-up procedures, and increased cost and anxiety for patients.

Our team has developed AutoParis-X (APX), an innovative tool designed to improve how urine cytology is analyzed. APX uses advanced computer algorithms to examine individual cells in urine samples and provide a clearer, more standardized assessment of cancer risk. This approach has the potential to streamline urine cytology in a manner similar to how automation improved Pap smear screening for cervical cancer.

Early studies supported by the National Institutes of Health have shown that APX performs well at a single medical center. This project seeks to prepare APX for broader clinical use by refining and validating it across multiple healthcare settings and by gathering input from clinicians and patient advocates about how such a tool should be used in practice. By addressing technical challenges and understanding real-world barriers to adoption, this work aims to improve the accuracy, efficiency, and consistency of bladder cancer screening.

Ultimately, this research has the potential to reduce the burden of surveillance on patients, improve early detection of recurrent disease, and support more cost-effective, patient-centered bladder cancer care.
