Bladder Cancer Survivorship Care Plans (SCPs)

The BCAN Survivorship Task Force—comprised of healthcare providers, patient advocates, and BCAN staff—has developed three unique survivorship care plan (SCP) templates based on expert input and current data. These plans are tailored for:

  • Non-Muscle Invasive Bladder Cancer (NMIBC)
  • Muscle-Invasive Bladder Cancer (MIBC)
  • Advanced Disease

These templates can be used as presented or customized to meet the specific needs of your institution and patients.

What Are Survivorship Care Plans?

Survivorship care plans (SCPs) for bladder cancer are individualized, comprehensive documents developed collaboratively by a patient’s care team. SCPs:

  • Guide post-treatment care
  • Help optimize long-term health outcomes
  • Serve as a communication tool between patients and providers
  • Ensure continuity of care across settings
  • Should be updated regularly as health status or guidelines change

Tips for Implementing SCPs in Clinical Practice

The BCAN Survivorship Task Force recommends the following strategies:

  • Designate a Champion: Identify a staff member to lead SCP implementation.
  • Start Small: Begin with a specific patient subgroup (e.g., MIBC) to create a workflow. This includes identifying who completes the SCP, who delivers it, and when.

  • Integrate with EMRs: Work with IT to incorporate SCPs into the electronic medical record system. This allows for automatic population of relevant data.

Note: There is no single “best way” to implement SCPs. Practices vary, and these resources are intended to support flexibility and customization.
